Vital Recuperative Care is a Columbia Heights recuperative care home licensed in Minnesota and operated by Vital Care Solutions LLC. Our house holds a small census on purpose — so every patient gets more than a bed and every referring team gets a real person on the phone.

Mission

Provide safe, compassionate recuperative care to Minnesotans — especially adults experiencing homelessness or housing instability — so they can heal after illness, injury, or surgery instead of cycling back through the emergency department.

Vision

A Minnesota where no hospital discharge ends at a bus stop, a shelter mat, or a friend's couch — and where recuperative care is a routine, expected part of the discharge plan for anyone who needs it.

Values

Dignity over intake speed. Clinical accuracy over paperwork. The patient's whole life — not just a diagnosis code. And a belief that small, well-run homes produce better outcomes than institutional beds.

Our recuperative care home sits on a quiet residential block in Columbia Heights — minutes from HCMC, Abbott Northwestern, Unity, Mercy, and the Minneapolis VA. It was chosen for its accessibility, not its proximity to an institution: patients heal better when the environment looks less like a hospital and more like where they'd want to stay.

Leadership

Built around a nurse-led intake.

Every referral passes through a licensed intake nurse before admission. Our operating team works closely with a Medical Director who oversees the plan of care for each patient and reviews any mid-stay changes. We also maintain direct lines of communication with the admitting hospital's discharge planner, social worker, and primary care provider for the duration of each stay.

Medical Director & Nursing Staff

A Medical Director supervises plans of care. RNs and LPNs are on-site or on-call 24/7. Every referral is reviewed by a licensed intake nurse before admission.
