OUR RECUPERATIVE CARE SERVICES

Our recuperative care services are designed to support healing in a safe, comfortable environment. Guests can expect:

• Daily checks with an RN/MD to ensure guests are recovering well
• Individual or shared sleeping spaces for privacy and rest
• Private-access shared bathrooms to ensure dignity and comfort
• Three nutritious hot meals daily to promote recovery and wellness
• On-site laundry facilities for convenience and cleanliness
• Compassionate, professional care delivered with respect and integrity

Individuals may qualify for recuperative care services if they meet all of the following requirements:

• Are enrolled in MHCP and are 21 years or older for Medical Assistance, or 19 years or older for MinnesotaCare
• Are currently homeless or without stable housing
• Require short-term medical care expected to last less than 60 days
• Need support with one or more of the approved recuperative services listed below

• Basic nursing support (e.g., tracking physical symptoms and managing pain)
• Wound care
• Help with medications
• Education related to the patient’s condition and care
• Immunization review and administration
• Setting clinical goals for recovery and discharge
• Coordinated care planning, including initial assessment of medical, mental health, and social needs
• Developing a personalized care plan
• Referrals and assistance with housing, legal support, transportation, case management, and social services
• Help accessing health coverage and other eligible benefits
• Monitoring and follow-up during the care period
• Medical, behavioral, and peer support services appropriate for a recuperative setting
• Services provided by community health workers (as defined in MN Statutes 256B.0625, subds. 49–50)

Recuperative care services are not covered under the following circumstances:

• The individual's mental or behavioral health needs exceed what can be safely managed in the recuperative setting
• The person cannot manage basic daily activities independently, such as standing up or using the bathroom without assistance

At our facilities, guests can recover with dignity in a calm, supportive setting. Our services are designed to help each guest rebuild health and confidence, and make a smooth transition to more stable living.
