OUR RECUPERATIVE CARE SERVICES

We provide short-term, supportive care for individuals recovering from illness, injury, or a recent hospital stay—especially those without stable housing. Our goal is to offer a safe, structured environment where guests receive essential health and social support as they work toward greater stability and independence.

Our Recuperative Care Services Include:

• Basic nursing support, including regular health checks and pain management
• Medication assistance and monitoring
• Health education and wellness coaching
• Review and updates of immunizations
• Development of personalized recovery goals and discharge planning
• Comprehensive assessment of medical, behavioral, and social needs
• Creation of individualized care plans
• Assistance with referrals for housing, legal aid, transportation, and case management
• Guidance on accessing healthcare coverage and other public benefits
• Ongoing care coordination and follow-up
• On-site medical, behavioral health, and peer support services
• Community Health Worker services (per Minnesota Statutes PS68.062, 43.25 subd. 4S, 43.26)
